Maintaining Your Benefits Coverage During Unemployment
If your coverage ends due to unemployment with recall rights, you’ll have the opportunity to maintain your coverage by making self-payments directly to LiUNAcare Local 506. Please ensure that your Union Dues with LiUNA Local 506 are up to date. Self-payments can be conveniently made using one of the following methods:
Step 1
Log into your online banking institution.
Step 2
Navigate to “Pay Bills” page.
Step 3
Select “Add Payee” and search for LiUNAcare Local 506 and “Select”.
Step 4
Under Account Number/Payment Reference, include the last 8 digits as displayed on your LiUNA Local 506 Benefit Card. Where to find your Benefit ID?
Your Benefit ID is located as the last 8 digits of your benefit card number on your Member Advantage Photo Card, located here:
Step 5
Select “Pay this Payee” and make your monthly benefit payment of $81.00, up to 3 months maximum; or as listed on your friendly reminder notice.
Step 6
You will receive a confirmation number when payment is made. Please keep this number for your reference.
Note:
Please allow 48 hours to process your payment.
Self-payments should be initiated within 31 days from the termination of your coverage and should be made regularly without any gaps. Please note that we do not accept retroactive self-payments.
Step 1
Make a cheque payable to “Local 506 Members Benefit Fund” in the amount of your monthly benefit payment of $81.00, up to 3 months maximum; or as listed in your friendly reminder notice
Step 2
Print your full name and your Member Advantage Benefit Card Certificate ID Number (last 8 digits) on the back of your cheque to ensure that your account is properly credited. Where to find your Benefit ID?
Your Benefit ID is located as the last 8 digits of your benefit card number on your Member Advantage Photo Card, located here:
Step 3
Place your cheque in an envelope and mail it to: LiUNACare Local 506 and the following address: 3750 Chesswood Drive, Suite 1, Toronto, ON M3J 2W6
