Coverage Details
What is covered?- Maximum combined benefit of $400 per calendar year for eyeglasses (lenses/frames combined) or Contact Lenses.
- One (1) eye exam within the same calendar year up to a maximum of $125. Eye exam prescriptions will be valid for 24 months from the date of exam.
- Corrective Laser Eye Surgery: $1,500 / once per lifetime.
